6. Latest Income Tax Form 'J'
7. Copy of EPF statement last received (if any)

Self Employed

1. Copy of the Sale & Purchase Agreement (2 sets)
2. Copy of the Land Title (if any)
3. Copy of I/C of Applicant(s) (2 sets)
4. Business Registration for Business/Company
5. Income Tax Form 'J' for past 3 years
6. Business and Personal Current Account statements for past 3 months
7. Audited Accounts of Company for past 2 years
